DD Waiver Registered Nurse (Part Time)

POSITION SUMMARY: Under the direction of the Director of Adult Services: Provide delivery of professional nursing services in compliance with the New Mexico Nursing Practice Act and in accordance within the framework of DDSD. Will be responsible for ensuring assigned health care services are provided/met for individuals under the DD Waiver, Mi Via Waiver and DOH Respite. Coordinates with individuals, families and guardians to ensure all appointments are up to date and timely. Complete required documentation into a data base, producing reports accurately and timely. Works with individuals with developmental disabilities of varying degrees; completes health care and emergency medical plans; developing, training, monitoring and revisions of those plans to support staff and family providers.

DUTIES AND RESPONSIBILITIES / ESSENTIAL JOB FUNCTIONS:

  1. Follow all nursing related responsibilities described in the DDSD Nursing policies,
  2. Over sight of all activities of LPN
  3. Utilize a computer data system developed to record all health and safety needs for individuals served, leading to documentation that must be generated, distributed and filed,
  4. Maintain documentation of all nursing activities – direct or indirect- in contact logs or progress notes,
  5. Assessment of individuals for basis on which the IDT determines that an individual’s needs nursing services, producing the documents for review by team members,
  6. Collaborates with the IDT, including participation in meetings as needed or required,
  7. Provide on-call supports as needed for the La Vida Felicidad DD Waiver individuals,
  8. Review of medical appointments, lab results, medications prescribes and reviews MARS for safety and accuracy, offering suggestions
  9. Assist with training needs for direct care staff on health care plans, medical emergency response plans,
  10. Assist in training direct care staff on care precautions ad practices related to the individuals specific medical care needs.
  11. Interact with team members in a positive and constructive way- team members include but are not limited to: Case Managers, Families, Guardians, Therapists, Health Care Professionals all agency 10-13 named duties
  12. Respond to the medical concerns as brought by the Customized Community Supports In-house supports during delivery of their services and supports,
  13. Travel within the State of NM to the homes of the individuals served

OTHER JOB RESPONSIBILITIES:

  • Attend trainings outside of La Vida Felicidad,
  • Offer specialized/extra trainings to other support staff within La Vida Felicidad,
  • Keep all documents required files appropriately,
  • Perform all other duties as assigned by the Director of Developmental Disabilities Services and/or the Executive Director.

SUPERVISORY RESPONSIBILITIES:

Directly supervises DD Waiver LPN

ROLE QUALIFICATIONS:

Education Requirements

  • Must be a New Mexico licensed Registered Nurse

Experience Requirements

  • 1 year experience as a Licensed Registered Nurse required
  • 6 months experience working with individuals with developmental disabilities preferred

Other Knowledge, Skills and Abilities

  • Must maintain confidentiality in accordance with HIPAA, LVF policy and state and federal law.
  • Demonstrable organizational skills, including time management.
  • Requires effective oral and written communication skills, excellent interpersonal skills, and computer literacy in word and excel.
  • Must maintain licensure as a RN as required
  • Requires ability to use a variety of office equipment: Fax, Copier, Mail, Computer, etc.
  • Bi-lingual a plus: English/Spanish, English/Native American languages
