Homecare Service Coordinator

POSITION SUMMARY: Provide oversight and expansion of services in the North and South Valley and in LVF private pay services. Provide oversight of services in Valencia for ALTSD and AAA (non-metro). Coordinate and provide home visits to homecare consumers, maintain consumer caseload and monitor provision of services to the assigned consumers.

DUTIES AND RESPONSIBILITIES / ESSENTIAL JOB FUNCTIONS:

  1. Coordinate oversight in the provision of LVF home care services in Bernalillo and Valencia Counties.
  2. Establish relationships and provide communication as needed with funding sources, case managers ALTSD contracts and referral services in assigned counties.
  3. Expand and implement increase services in the south valley and surrounding area to better utilize the City of Albuquerque contract.
  4. Coordinate and provide home visits to home care consumers quarterly/semi-annually as required by funding source.
  5. Monitor and provide documentation on consumer safety and general physical health and well-being and continual need for assistance.
  6. Knowledge and compliance with policies and procedures of funding source and LVF.
  7. Coordinate referral and intake process for home care department; assist with annual renewals.
  8. Provide advocacy for consumers on as needed basis.
  9. Schedule direct care providers to provide services for consumers and provide substitutes as needed. Conduct performance reviews of supervised providers annually.
  10. Make recommendations to hire/terminate direct care providers.
  11. Assist Homecare Manager with the compilation and completion of Focus on Excellence Reports
  12. Responsible for follow up/corrective action needed as a result of Quality Assurance team reviews
  13. Maintain current CPR/FA and other trainings/certification as required.
  14. Maintain confidentiality in accordance with HIPAA, LVF policy and state and federal law.
  15. Active Health and Safety Committee member

OTHER JOB RESPONSIBILITIES:

  • Maintain homecare database (ERSP)
  • Assist with maintenance system for tracking employee information.
  • Assist with filing of provider and client documentation.
  • Assist with training of direct care providers in the home care department.
  • Perform other duties as assigned by the supervisor  

SUPERVISORY RESPONSIBILITIES: Directly supervises direct care providers

ROLE QUALIFICATIONS:

Education Requirements

  • High School diploma or equivalent.

Experience Requirements

  •  Minimum one year experience with senior services or similar combination of education and experience.
  • Supervisory experience a plus.

Other Knowledge, Skills and Abilities

  •  Demonstrate organization skill, including time management.
  • Requires current knowledge of principles and practices of senior services programs.
  • Requires effective oral and written communication skills, excellent interpersonal skills, and computer skills.
  • English/Spanish bi-lingual a plus.

WORKING CONDITIONS:

  • Manual dexterity needed for computer and phone work..
  • The physical environment requires the employee to work primarily inside, in temperature controlled conditions. Out of the office and in the field work is also required. Some noise and interruptions are present.
  • Must be able to manipulate and operate office supplies and equipment.  Occasionally required to lift up to 50 pounds
  • An automobile is required; a valid, non-suspended NM driver’s license and a current automobile liability policy and continuous coverage.
  • Must pass a pre-employment background check.
  • No substantiation or criminal conviction of child abuse or neglect.
  • Travel within the state may be required.

Please note: this position is based in Los Lunas and requires driving to client locations in Valencia county. Current Driver’s License and proof of auto insurance required.
