Position Title: Family Living Provider

Qualifications:

• Must be approved as a direct support provider by Developmental Disabilities Support Division (DDSD) in accordance with the Medicaid Home and Community Based Waiver Service Standards for the Developmentally Disabled.
• Must be able to follow and implement the DD Waiver Service Standards.

Requirements:

• Must be 21 years of age or older,
• Have a high school diploma or GED (some exceptions are allowable),
• Clear Criminal Record Background Check and Adult/Child Abuse Screening,
• Proof of Homeowners insurance/renters’ insurance,
• Appropriate private bedroom for individual served,
• Proof of Vehicle Liability Insurance and Valid NM Driver’s License,
• Successful completion of all required training, including CPR, First Aid, and all other required training within allowed

La Vida Felicidad, Inc Service Coordinator under the direction of the Director of Adult Services

The State of NM DOH/DHI/QMB/DDSD may monitor and visit, as needed, as scheduled and/or unscheduled.

Position Definition:

Provide Family Living service/supports 24 hours a day, 365 days a year.

Typical Physical Demands:

Good General health with the ability to lift 50 lbs. And unlimited range of motion. Able to drive a vehicle/provide or arrange transportation.

Duties and Responsibilities:

Community Living Services include, but are not limited to the following:

1. Assistance with money
2. Meal planning and preparation
3. Routine household
4. Community integration/access/utilization.
5. Promote, develop, and maintain social, spiritual and individual
6. Implement behavior support and crisis
7. Develop and provide recreation and leisure time
8. Development and maintenance of recreation/leisure/exercise
9. Training and education on self-advocacy, and sexuality. I 1 Individual health maintenance and monitoring
10. Individual health maintenance and monitoring
11. Arrange for transportation of individual to medical and dental
12. Transportation and/or arrangement of transportation for, but not limited to:
    1. Day Service or employment
    2. Leisure and recreation activities or services
    3. Medical appointments
13. Personal care or activities of daily living (such as bathing, eating, dressing, and individual hygiene).
14. Remind/observe/monitor medication and pharmacy
15. Provide assistance with self-administration of
16. Nutritional
17. Assistance to individuals who require a wheelchair for mobility and need physical assistance for bathing, dressing, and transfers.
18. Implement activities in support of therapy
19. Provide coordination of all healthcare in accordance with the guidelines of the healthcare coordinator as outlined in DD Waiver Standards.
